Incident summaries that operations will reopen

Operations teams reopen documents when they trust the author understood constraints. We teach PMs to open with a neutral timeline, then separate observations from decisions already made.
We also drill on verbs. 'We should consider' reads like avoidance after an outage. 'We will' reads like a promise without owners. The middle path is a crisp action table: owner, date, dependency.
Participants bring redacted samples. Coaches mark up two versions: one safe for wide circulation, one detailed for the technical guild. Seeing both versions side-by-side helps writers choose the right envelope.
Finally, we rehearse live read-throughs. If the facilitator stumbles on a sentence, that sentence gets rewritten. Plain language is a courtesy to tired on-call engineers.
